Conscientous Objector

Name, a conscientous objector is an individual with a firm, fixed and sincere objection to the participation in war in any form or to the performance of military service because of religious training or beliefs.  Does this statement describe you?

FORMS/PROCESSING

 

In order to continue your processing we need to complete a series of forms that will begin your official Air Force Personnel Records.  It is very important that we complete these forms accurately. Building a solid and firm application now will prevent any delays in your processing.

Professional Relationship Form (recruiters stack)

My job as an AF recruiter is to provide you with the highest customer service possible, while creating an atmosphere free of discrimination and sexual harassment.  Our relationship is strictly professional and will remain so throughout your entire recruting process. This form details the Recruiter and Applicant conduct and professional relationship policy. I need you to read both sections of this form, please print and sign with your payroll signature (first name, middle intial, last name and any additions to your name) and date with todays date in military format DD MMM YY ( hand applicant a black pen)

AF Applicants Rights and Resp Card (applicants stack)

here is the af applicants rights and responsibity card. It provides you with phone numbers and information on professional conduct.  I will put this in your stack to take home with you, should you ever need it.

Credit Disclosure notice (recruiters stack)

Name, this is an authorization for release of information. Your signature gives the AF permission to procure a credit report for employment purposes from any consumer reporting agency.  Please sign with your payroll signature and date in military format ( explain)

medical prescreen of medical history report (DD 2807-2)

(recrutiers papers)

this is the medical prescreen of medical history report.  it is designed for you to dscribe your medical history. I will complete part I and i need you to complete part 2, items 1 through 75, by putting your initials (first, middle, last and any additions) in the appropraite response. Any item you intial yes give a brief explanation in item 2b. For example if you answer yes to item 1 indicate #1 in section 2b and include all information

next i need you to complete items 3 through 6 by filling in the names, addresses, telephone numbers, and insurance ID numbers requested for your primary care physcians and insurance providers

Finally, i need you to read the applicant certification area. Once you've read and understand it, then sign your payroll signature and date in the year, month, day format. For example todays date it _____ (YYYYMMDD)

USAF drug and alcohol abuse certificate (AF Form 2030)

you said earlier that you had/had not experimented with drugs. I need you tto attest to that statement on the USAF druvg and alcohol abuse certificate. It explains the Air Force's policy on drug and alcohol abuse.  I need you to the read the definition of terms, next complete the certification at time of applicant, by placing your initials in the appropriate blocks. (first, middle, last, and any addtions)

Afterwards, read and initial the statements of understanding once you've completed all areas sign with your payroll signature and date in military format (explain).

MEPCOM FORM 680 (depends if applicant has tested or not whose stack this form goes into)

This is the MEPCOM form 680, request for examination. It is a simple request for you to take the writtem examination. (or to pull the scores if applicant has already tested) I will complete the top half for you. On the bottom is the applicant certification in presence of recruiting personnel. I need you to read the applicant certification "X" the appropriate block, (explain options), and provide the necessary information.  Then sign with your payroll signature and date in military format (explain)

this test is given on monday through thursday at 6 pm. Which day is better for you?

Here are the directions to the test site. You'll need to bring a picture ID and your social security card with you.

discrimination and sexual harassment forms - 2 copies

earlier we discussed the AF's policy on professional relationships. Next, i have the AF discrimination and sexual harassment forms. They are designed to assist you in understanding the AF's policy on discrimination and sexual harassment. I need you to read the AF's policy section and then read the policy regarding Recruiter-Recruit relationships. Finally, read your rights as an applicant, then sign both copies with your payroll signature and date in mil format (explain).

Applicant Profile Worksheet

this is the applicant profile worksheet. I will need you to bring some documents back with you at the time of our next appointment. These documents need to be original or certified copies. (simulate marking the appropraite items needed)

The rest of this form is like a job application. Complete the front and the back, and bring it back with you at the time of your next appointment.

MEPS

A very important part of your processing happens at the Military Entrance Processing Station (MEPS). It's an all day appointment beginning at 0600.  It is at MEPS that you will complete the necessary physcial required and your secrity background check for entrance into the Air Force will begin.  Once you complete and pass the physical, you will sit down with an AF job counselor, who will assist you in obtaining a job or skill area. Once you have completed all these steps you will then swear into the Delayed Entrance Program or DEP.
