Chapter 4: The Shoulder Girdle

Chapter 4: The Shoulder Girdle

S. D.
This flashcard set delves into the intricate kinesiology of the shoulder girdle, focusing on muscles like the pectoralis minor, serratus anterior, and trapezius, along with their actions, origins, insertions, and innervations. It covers key movements such as rotation, elevation, and depression, and explores joints like the acromioclavicular and sternoclavicular. Ideal for university students studying medical anatomy, this set provides a detailed understanding of shoulder mechanics, benefiting those aiming to master the complex interplay of muscles and bones in the shoulder region.

Anterior Shoulder Girdle: Key Bony Landmarks

  • Manubrium
  • Clavicle
  • Coracoid Process
  • Acromion Process
  • Glenoid fossa
  • Scapula

Posterior Key Bony Landmarks of Shoulder Girdle

  • Lateral border
  • inferior angle
  • medial border
  • superior angle
  • spine of the scapula

Shoulder Girdle (scapulothoracic) joint

scapula moves on the rib cage

joint motion occurs at sternoclavicular joint & acromioclavicular joint

Scapulothoracic Joint

  • not a true synovial joint
  • does not have regular synovial features
  • supported dynamically by its muscles
  • no ligamentous support

Sternoclavicular (SC)

  • arthrodial classification
  • ligaments
    • anterior & posterior SC ligament
    • Costoclavicular ligament prevents superior displacement
  • the clavicle moves anteriorly 15 degrees with protraction and moves posteriorly 15 degress with retraction

Acromio-clavicular (AC)

  • arthrodial classification
  • supported by CC & AC ligaments
  • often injured

Abduction (protraction)

scapula moves laterally away from spinal column

Transverse plane

Adduction (retraction)

scapula moves medially toward spinal column

tranverse plane

Upward Rotation

  • moving glenoid fossa & inferior angle of scapula upwards & outwards
  • frontal plane

Downward rotation

returning inferior angle toward spinal column

frontal plane

Elevation

  • superior movement
  • shrugging shoulder
  • frontal plane

Depression

  • downward or inferior movement
  • fontral plane

Trapezius Muscle- Upper Fibers

  • Action:
    • Elevation- frontal axis
    • Upward rotation frotal axis
    • extension of head at neck sagittal
    • rotation of head at neck transverse
  • Origin: base of skull, occipital bone, posterior liagments of neck
  • Insertion: posterior aspect of lateral 3rd clavicle
  • Innervation: spinal accessory nerve and branches of C3 and C4

Trapezius Muscle: Middle Fibers

  • Action:
    • elevation- frontal
    • adduction- transverse
    • upward rotation- frontal
  • Origin: spinous process of C7 & T1-3
  • Insertion: medial border of acromion process & upper border of scapular spine
  • Innervation: spinal accessory nerve and branches of C3 and C4

Trapezius Muscle: Lower Fibers

  • Action:
    • Depression- frontal
    • retraction- transverse
    • uowards rotation- frontal
  • Origin: spinous process of T4-12
  • Insertion: triangular space at base of scapular spine
  • Innervation: spinal accessory nerve and branches of C3 and C4

Levator Scapulae

  • Action:
    • elevation: frontal
    • weak doward rotation
    • weak retraction
  • Origin: transverse process of C1-4
  • Insertion Medial border & superior angle of scapula
  • Innervation: Dorsal scapula nerve, branches of C3-5

Rhomboid Major/Minor

  • Action:
    • Retraction: transverse
    • Downward rotation: frontal
    • elevation: frontal
  • Origin: spinous process of C7&T1-5
  • Insertion: medial borderof scapula below spine of scapula
  • Innervation: dorsal scapular nerve (C5)

Serratus Anterior

  • Action:
    • protraction: transverse
    • upward rotation: frontal
  • Origin: surface lateral ribs 1-9
  • Insertion: anterior aspect of medial border of scapula
  • Innervation: long thoracic nerve (C5-7)

Pectoralis Minor

  • Action:
    • protraction- transverse
    • downward rotation: frontal
    • depression: frontal
  • Origin: anterior surface of ribs 3-5
  • Insertion: Anterior aspect of whole length of medial border of the scapula
  • Innervation: Medial pectoral nerve (c8-T1)
