What are the 3 portions of the temproal bone and which portion makes up the TMJ?
- Squamous portion
- Petrous portion
- Tympanic portion: makes up the TMJ
Where does the temporal bone articualte with the mandible? Where does the mandible articulate with the temporal bone?
- Articular eminence and the articular fossa
- Head of the condyle
This completely encloses the TMJ.
Joint capsule
This is located between the articular eminence and the condyle.
Joint disc
These are formed by the joint and contain synovial fluid.
Synovial cavities (2)
What are the 3 ligaments associated with the TMJ?
- TMJ ligament: reinforces the joint capsule and prevents retraction (on lateral side)
- Sphenomandibular ligament: vestige of Meckel's cartilage. it become taught when jaw is protruded. It is a landmark for local anesthesia
- Sthylomandibular ligament: formed from cervical fascia.
What are teh 2 movements of the TMJ?
- Gliding: occurs between the disc and the articular eminence of the temporal in the upper synocial cavity to produce protrusion and retraction
- Rotation: occurs between the disc and the condlye in the lower synovial cavity accomplishing depression and elevation of the jaw.
What movements are accompanied with opening and closing the mouth?
- Opening: protrusion & depression
- Closing: retraction & elevation
This is when the head of the condlye moves too far anterior on the articular eminence. It can involve either one or both sides. Occurs when opning too wide (i.e. prolonged dental work).
Sublaxation
What are symptoms of TMD?
- Headaches not helped by migraine medication, especaially on rising or late in the afternoon
- Facial pain
- Ear pain or stuffiness in the absence of infection
- Pain and/or sounds in the TMJ
- Pain in the jaws, espeically when chewing or upon arising
- Limited and/or painful jaw opening or deviation to one side when opening
- A feeling that your bite is "off"
- Persistent neck pain, especially at the base of the skull, following a motor vehivle or sports accident
- Generlized tooth sensitivity, especially when chewing
